Outcomes of Osteochondral Allograft Transplantation for Femoral Head Cartilage Lesions: Minimum 2-Year Follow-Up

Purpose: To investigate the survivorship, radiographic, and clinical outcomes of femoral head osteochondral allografts (OCAs) in young active patients.

Summary:

  • A total of 29 patients were included in this study.
  • Graft survivorship was 78.4% and 62.7% at 5 and 10 years, respectively.
  • Ten patients (34.5%) progressed to a total hip arthroplasty.
  • At final follow-up, 69% of patients had an improvement in the modified Harris Hip score, with arthritic progression only seen in 26.9% of patients.
  • The preoperative diagnosis of osteonecrosis (ON) resulted in a significant decrease in graft survivability, with a 10-year survival of 41.8% with an ON diagnosis and 85.7% graft survivability without ON.


JRF Ortho Take Away: With a 5-year survival rate of 78.4% and a 10-year rate of 62.7%, osteochondral allograft (OCA) transplantation of the femoral head is an effective treatment for young, active patients. Identifying suitable candidates and accurate preoperative diagnoses can enhance outcomes and graft survivorship.
