Bone Marrow Aspirate Concentrate May Decrease Reoperation in Osteochondral Allograft Transplantation: A Prospective, Randomized, Double Blind Investigation

Purpose: To evaluate whether adding bone marrow aspirate concentrate (BMAC) to osteochondral allograft transplantation improves graft integration, patient-reported outcomes, or reduces reoperation rates compared to standard allografts. 

Summary:

  • This prospective, randomized, double-blind trial found patients who received BMAC treated OCAs were less likely to need subsequent surgeries for graft debridement or revision. 
  • OCAs treated with BMAC showed a trend toward reduced large cyst formation and were more likely to show small cystic changes. 
     

JRF Takeaways: BMAC is a biologic adjunct that may enhance outcomes following osteochondral allograft (OCA) transplantation. In this study, the addition of BMAC was associated with a reduced need for subsequent debridement and revision surgeries. Postoperative imaging revealed a greater incidence of small cystic changes in the BMAC group, with a trend toward reduced formation of larger cysts. Patients who received BMAC also demonstrated a trend toward improved KOOS JR scores at both 1- and 2-year follow-up. Further studies with larger sample sizes are warranted to further elicit these differences.
